Key Responsibilities:- Conduct a range of case management tasks, including performing psychosocial assessments, creating individualized service plans, and preparing regular updates on client progress.
- Educate clients about program guidelines and assist them in navigating the service plan development process.
- Ensure meticulous documentation and uphold the confidentiality of client information.
- Connect clients with external organizations and resources as necessary.
- Support clients in establishing relationships with outside services and coordinate transportation as needed.
- Lead workshops and group sessions with clients to address social service objectives and individual needs.
- Bachelor's Degree in Human Services or a related field from an accredited institution.
- Prior experience working with vulnerable populations or similar service programs is preferred.
