Job Overview

Position: Case Manager

Department: Supported Housing

Reporting To: Program Director

Work Schedule: Full-time - 35 hours per week; evening and weekend hours may be necessary

Salary: $50,000

The Sea Rise Supportive Housing Program encompasses a total of 199 units, with 72 designated for supportive housing. Among these, 35 units cater to individuals with serious and persistent mental health conditions, 20 units are allocated for homeless young adults aged 18-25, and 17 units are intended for individuals with intellectual and/or developmental disabilities. BCS provides essential on-site services, including case management and various support services.

Key Responsibilities:

  • Assist clients in accessing a range of services, including specialty care referrals (medical, mental health, substance abuse), appointment coordination, and other necessary processes to support residents in achieving their Individualized Treatment Plan goals and enhancing their self-efficacy.
  • Timely completion of all required assessments and service plans.
  • Document progress notes within 24 hours following individual sessions.
  • Conduct weekly meetings with all assigned clients or as directed by the Program Director.
  • Ensure accuracy and timeliness in completing all clinical and administrative documentation in compliance with program policies and DOHMH standards.
  • Provide crisis intervention services and prepare incident reports in line with DOHMH regulations.
  • Work collaboratively with residents in their homes and within the community to improve daily living skills, including apartment upkeep, personal hygiene, meal preparation, budgeting, and navigating care systems.
  • Accompany clients to appointments as needed, including medical and psychiatric visits.
  • Collaborate with property management to address client housing needs and maintenance issues.
  • Conduct regular apartment inspections as required.
  • Assist clients in navigating the recertification processes for their apartment units.
  • Provide case management coverage during program absences to maintain care quality.
  • Facilitate weekly group sessions on topics such as mental health awareness and substance abuse prevention.
  • Participate in fire drills and community recreational events to enhance socialization skills.
  • Support community residents and church social programs.
  • Attend all required meetings, trainings, and case conferences.
  • Complete additional tasks as assigned by the Program Director.

Qualifications:

  • Bachelor's degree in a human services-related field, with at least two years of experience working with homeless youth in supportive housing settings.
  • CASAC or CASAC-T certification preferred.
  • Strong written and verbal communication skills.
  • Basic proficiency in computer and data entry tasks.
  • Background check required due to direct client interaction.
