Job Overview

Position Title: Team Lead

Reports To: Program Director

Position Summary:

The Team Lead is responsible for overseeing and guiding staff while coordinating comprehensive services for individuals supported by the program. This role involves close collaboration with program leadership and clinical professionals to empower individuals to achieve their desired life outcomes, reflecting a commitment to the core values of Resources for Human Development.

Key Responsibilities:

Staff Oversight
  • Provides direct supervision to care staff, fostering their professional growth through coaching and mentorship.
  • Organizes and allocates tasks among team members effectively.
  • Ensures staff are informed about relevant policies and procedures impacting their roles.
  • Implements organizational policies as outlined by Resources for Human Development.
Direct Support
  • Maintains a manageable caseload while delivering direct services.
  • Coordinates medical appointments, ensuring effective communication with healthcare providers regarding the needs of individuals.
  • Fosters positive relationships with families, guardians, and other stakeholders to address policies and concerns.
Administrative Duties
  • Actively engages in the person-centered planning process, including:
    • Attending planning meetings
    • Providing valuable input
    • Coordinating training plan development
    • Ensuring documentation is thorough and timely
    • Reviewing progress and daily notes
    • Monitoring plan implementation
    • Conducting monthly reviews
    • Communicating with service coordinators
  • Prepares and submits necessary reports related to individuals supported.
  • Ensures compliance with all regulatory standards.
  • Participates in continuous quality improvement initiatives.
  • Performs additional duties as assigned.

Qualifications:

  • Bachelor's Degree in a Human Services field (e.g., sociology, special education, rehabilitation, nursing, counseling, psychology) OR
  • An Associate Degree with three years of relevant experience OR
  • A High School Diploma with five years of relevant experience.
  • Current Certified Psychiatric Rehabilitation Practitioner (CPRP) Certification.
  • Valid Driver's License.

Working Conditions:

Environmental Hazards:
  • Exposure to fumes, dust, extreme temperatures, and loud noises.
  • Potential exposure to mechanical, chemical, and electrical hazards.
Physical Requirements:
  • Ability to lift up to 50 pounds occasionally and perform various physical tasks including standing, walking, climbing, and repetitive motions.
