Job Overview

Position Title: Team Lead

Reports To: Program Director

Position Summary:

The Team Lead is responsible for overseeing and guiding staff, ensuring the delivery of comprehensive services to individuals within the program. This role requires close collaboration with program management and both clinical and direct support teams to empower individuals to achieve their desired life outcomes. Key Responsibilities:

Staff Management:

  • Provides direct supervision to care staff, fostering their professional growth through coaching and mentoring.
  • Organizes and allocates tasks among team members effectively.
  • Ensures staff are well-informed about relevant policies and procedures.
  • Implements organizational policies as established by Resources for Human Development.

Direct Support:

  • Maintains a direct service caseload.
  • Coordinates and oversees medical appointments, including attending when necessary, and communicates with healthcare professionals regarding client needs.
  • Fosters positive relationships with family members, guardians, and staff, addressing any concerns or issues.

Administrative Duties:

  • Actively engages in the person-centered planning process, which includes:
  • Participating in planning meetings,
  • Providing valuable input,
  • Coordinating training plan development,
  • Ensuring all documentation is accurate and complete,
  • Reviewing progress and daily notes,
  • Monitoring plan implementation,
  • Conducting monthly reviews, and
  • Communicating with service coordinators.
  • Prepares and submits necessary reports and documentation related to individuals served.
  • Ensures compliance with all regulatory requirements.
  • Implements continuous quality improvement initiatives.
  • Performs other related duties as assigned.

Qualifications:

  • Bachelor's Degree in a Human Services field such as sociology, special education, rehabilitation, nursing, counseling, or psychology; OR
  • An Associate Degree with three (3) years of experience in supporting individuals with intellectual disabilities; OR
  • A High School Diploma with five (5) years of relevant experience.
  • Current Certified Psychiatric Rehabilitation Practitioner (CPRP) Certification.
  • Valid Driver's License.

Working Conditions:

Hazard and Atmospheric Conditions:

  • Exposure to fumes, dust, extreme temperatures, loud noise, and various mechanical and chemical hazards.

Physical Requirements:

Lifting Requirements:

  • Medium: ability to exert up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds constantly to move objects.

Physical Abilities:

  • Ability to stand or sit for extended periods.
  • Capable of walking and using hands for various tasks.
  • Ability to climb stairs or ladders.
  • Effective communication skills, both verbal and auditory.
  • Visual acuity for tasks requiring detection and assessment.
  • Ability to perform repetitive motions and reach in various directions.
  • Ability to kneel, stoop, crouch, or crawl as necessary.
