Position Overview
As a Care Navigator, you will play a crucial role in formulating, evaluating, and orchestrating care management initiatives tailored to the needs of our members, ensuring optimal and cost-efficient healthcare results.

Key Responsibilities
Your primary responsibilities will include:

  • Creating individualized care/service plans.
  • Providing education to members and their families/caregivers regarding available services and benefits.
  • Advocating for enhanced access to healthcare resources.
  • Facilitating communication between members, their families/caregivers, and the healthcare provider team to guarantee timely access to necessary care or services.

Additionally, you may be tasked with conducting in-person assessments of members' needs, collaborating with healthcare providers or resources, and offering guidance to care managers and/or members and their families/caregivers on procedures, healthcare provider instructions, care alternatives, referrals, and healthcare benefits.
