Job Summary

The Palliative Care Nurse Coordinator is a vital role at Del Sol Medical Center, where we prioritize patient-centered care and compassion. Key Responsibilities
  • Provide guidance and support to healthcare team members in delivering palliative care services
  • Develop and implement a palliative care program that meets the needs of patients and families
  • Collaborate with interdisciplinary teams to ensure seamless care coordination
  • Provide education and training to healthcare staff on palliative care best practices
Requirements
  • Texas Registered Nurse licensure or Compact State Registered Nurse licensure
  • BCLS certification
  • Associates or Bachelor's Degree in Nursing
  • Hospice and Palliative Care Nursing experience with a proven track record in leadership, education, team building, program development, and interdisciplinary collaboration
Preferred Qualifications
  • RN from an accredited school of nursing
  • BSN preferred
  • Certified Palliative and Hospice Care Nurse (CHPN) certification
  • Critical care experience in a hospital setting
