Position Overview
As a Care Navigator, you will play a crucial role in the development, evaluation, and coordination of care management initiatives tailored to the needs of our members, ensuring effective and high-quality healthcare outcomes.
Key Responsibilities
Your primary responsibilities will include:
- Creating individualized care/service plans.
- Educating members and their families/caregivers about available services and benefits.
- Advocating for enhanced access to healthcare services.
- Facilitating communication between members, families/caregivers, and the healthcare provider team to guarantee timely access to necessary care or services.
Additionally, you may be tasked with conducting in-person assessments to evaluate member needs, collaborating with healthcare providers or resources, and providing educational support to care managers and/or members and their families/caregivers regarding procedures, provider instructions, care alternatives, referrals, and healthcare benefits.
