We are seeking a highly motivated and compassionate Registered Nurse to join our Virtual Care team. As a key member of our interdisciplinary care team, you will play a critical role in providing telephonic triage and directional patient care advice to our patients and their families.
Key Responsibilities:- Provide telephonic triage and directional patient care advice to patients and their families
- Collaborate with the interdisciplinary care team to develop and implement comprehensive care plans
- Conduct remote clinical assessments and provide health education and counseling to patients and their families
- Monitor patient health and provide support and guidance to patients and their families
- Associate Degree in Nursing required
- Bachelor's Degree in Nursing preferred
- Nurse Licensure Multi-state Compact license required
- Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required
- Minimum of 2 years clinical nursing experience working with Geriatric patients in an Emergency, Care, or Trauma Care setting
