Position Overview
As a Care Coordinator, you will play a pivotal role in the development, evaluation, and management of care activities tailored to the unique needs of each member, ensuring effective and economical healthcare results.
Key Responsibilities
Your primary responsibilities will include:
- Creating individualized care plans and service strategies.
- Providing education to members and their families about available services and benefits.
- Advocating for enhanced access to healthcare resources.
- Facilitating communication between members, their families, and the healthcare provider team to guarantee prompt access to necessary services.
Additionally, you may be tasked with conducting assessments through on-site visits to better understand member needs, collaborating with healthcare providers, and offering guidance to care managers and families regarding procedures, care options, referrals, and healthcare benefits.
