Role Summary: As an Ophthalmic Care Coordinator, you will play a crucial role in assessing and managing the care needs of patients facing chronic eye conditions. Your responsibilities will include collaborating with patients, their families, healthcare professionals, and internal teams to create and implement individualized care strategies tailored to each patient's unique needs.
Key Responsibilities:
- Engage with patients and their support systems to discuss care aspects and address concerns.
- Coordinate with home health agencies and community resources to ensure comprehensive care.
- Collect and document health outcomes data for ongoing assessment.
- Educate patients and caregivers on self-management and daily living activities.
- Support adherence to treatment plans and medication management.
- Identify and facilitate access to community and health resources.
- Conduct regular reviews of patient status, including laboratory results.
- Develop and maintain comprehensive care plans specific to individual diseases.
Qualifications:
- A minimum of 2 years of experience in the eye care field.
- Certification as an Ophthalmic Technician (COA or COT/COMT) is required.
- Familiarity with Electronic Medical Record (EMR) systems is preferred.
- Ability to work independently in a remote setting with minimal supervision.
- Exceptional verbal and written communication skills.
- Strong organizational abilities and customer service orientation.
- Reliable internet access and a suitable home office environment compliant with HIPAA regulations.
