Registered Nurse-Mammography Coordinator

As the Mammography Coordinator (RN) you will function within the Women Veteran Program and coordinate care throughout the El Paso VA Healthcare System, Las Cruces CBOC, and East El Paso CBOC as well as with William Beaumont Army Medical Center.
You will collaborate with these other entities to assure quality care is provided to veterans. You must demonstrate excellent intrapersonal communication skills. Display excellent team collaboration with the ability to work with internal and external stakeholders. You will report to the Women Veteran Program Manager. You must possess strong organizational skills as well as a demonstrated ability to assess and prioritize workload. Must be competent to function independently and exercise sound decision making. Proficient in Excel and other tracking software tools. Familiar with the basic statistical data analysis as well as generating reports.Key roles include but are not limited to:
Function as a nurse navigator by serving as a liaison between the patient, the non- VA provider, and the VA facility.
Monitor the delivery of care, coordinating such care (documentation, patient notification, screening/follow up care, etc.) and tracking outcomes of services that have been furnished through purchased care.
Work collaboratively with staff from other services within and outside the medical center, including VHA facilities, Military Treatment Facilities, and various Non-VA (community) facilities.
Ensure seamless transition between facilities and levels of care. Facilitate access to local and community resources. Facilitate scheduling of necessary follow-up imaging or testing as required. Conduct trending and tracking of veterans receiving mammography screenings through Breast Care Registry or other tools.Review and track all non-VA care consults entered for breast imaging, follow up on these consults to ensure timely scheduling of patients with non-VA providers.
Ensure that all breast imaging reports or cancer diagnosis reports are received and reviewed by health professionals. Track abnormal results and actions taken until completion of needed follow up. Ensure that patient notification has been documented in CPRS chart.Works with Women's Health Leadership to communicate and coordinate with the PACT teams to develop treatment plans for women veterans.
