Senior Care Coordination Specialist

Key Responsibilities:
As a Senior Care Coordination Specialist, your primary responsibilities will include:
- Managing patient admissions and overseeing the department's financial transactions.
- Tracking referrals to specialists and coordinating physical therapy appointments.
- Handling moderately complex assignments that require sound judgment and problem-solving skills.
- Prioritizing and distributing incoming communications, including mail and messages, to the appropriate personnel.
- Compiling and preparing statistical reports for management review.
- Organizing special events and interpreting policies to ensure compliance with confidentiality standards.
- Collecting and verifying patient information for billing and charting purposes.
- Setting up payment plans in accordance with hospital policies and assisting patients with necessary documentation.
- Ensuring timely verification of third-party payer accounts and explaining financial obligations to patients.
- Maintaining employee education records and managing various administrative tasks.
Qualifications:
To be successful in this role, candidates should possess:
- A high school diploma or GED; an Associate's or Technical Degree in a related field is preferred.
- Relevant experience in a healthcare setting, ideally two years or more.
