Role Summary:

The Lead Medical Respite Coordinator is responsible for supervising the Medical Respite Coordinators and Navigators, ensuring efficient care coordination, client assistance, and collaboration with multidisciplinary teams. This position also manages a caseload and plays a vital role in guiding clients through the healthcare system, overcoming obstacles to permanent housing, and achieving favorable client results. The standard working hours are Monday to Friday, 8:30 AM to 5 PM.

Key Responsibilities:

  • Oversee the coordination of all medical and social services for clients post-discharge from hospitals and the medical respite facility.
  • Facilitate onsite telehealth appointments for both mental and physical health services.
  • Manage medication deliveries from local healthcare facilities and pharmacies.
  • Identify and connect clients with suitable local community resources.
  • Coordinate food services provided to patients.
  • Assist patients in transitioning to their next housing arrangements.
  • Engage actively in daily interdisciplinary care team discussions.
  • Maintain respectful and professional communication with all care team members to ensure patient-centered care.
  • Act as an advocate for patients, serving as a liaison between patients/families and community service organizations.
  • Support organizational initiatives aimed at fostering a positive workplace culture.
  • Uphold patient confidentiality and treat all patients with dignity and respect.
  • Perform additional duties as assigned by the supervisor or as outlined in program descriptions.
  • Provide leadership and guidance to the Medical Respite Coordinator and Navigator.
  • Conduct regular performance assessments and offer constructive feedback.
  • Facilitate training and professional development for team members.
  • Ensure adherence to organizational policies, procedures, and regulatory standards.
  • Promote a collaborative and supportive team atmosphere.
  • Oversee daily operations of the Medical Respite Program, including staffing and scheduling.
  • Collaborate with other supervisors and departments to enhance program outcomes and resource use.
  • Serve as a resource and escalation point for staff and client-related matters.

Candidate Qualifications:

  • Bachelor's degree in social work, psychology, sociology, public health, or a related field, or Licensed Practical Nurse, or 3 years of experience in case management, mental health, or a related area.
  • Limited License (if applicable).
  • Full License (preferred).
  • Minimum of 2 years' experience in case management and/or care coordination services.
  • Experience working with underserved populations.
  • Familiarity with identifying, accessing, and referring to local community resources.
  • At least 1 year of supervisory experience (preferred).

Additional Skills:

  • Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook).
  • Ability to learn new software applications.
  • Proficiency in operating general office equipment (PC, printer/fax/copier, telephony systems).
  • Experience with electronic health records and scheduling systems.
