Position Summary:

The Care Coordinator plays a vital role in providing administrative support to the case management team within the Minnesota Army National Guard, focusing on Individual Medical Readiness.

This non-clinical position requires personnel who are well-versed in all aspects of administrative functions and are qualified in their respective areas.

Compensation:

Hourly Rate: $34.22

Key Responsibilities:
  1. Monitor the medical status of all assigned Soldiers as directed by the case manager, including tracking appointments and documenting communications with Soldiers, their units, and military providers.
  2. Participate in case reviews to ensure effective tracking and management of Soldiers with open cases.
  3. Assist Soldiers in scheduling appointments with appropriate medical and dental facilities, and communicate with government personnel to ensure compliance.
  4. Oversee designated medical readiness databases to confirm that all scheduled services are completed and that current medical statuses are accurately reflected.
  5. Support Case Managers by assembling and processing various types of packets, as well as entering and tracking patient information.
  6. Receive, communicate, and process medical and personnel records submitted through various electronic systems.
Qualifications:

Education/Experience: A minimum of a high school diploma or equivalent is required, along with at least two years of experience in an administrative role, preferably in a medical setting.

Security: Candidates must pass a national security background check for access to military electronic systems and demonstrate proficiency in MS Office Suite and general administrative tasks.
