Job Description:

The Clinical Care Nurse (LPN) - Clinic plays a vital role in supporting healthcare providers by delivering timely, quality patient health services in a clinical setting. Responsibilities may include gathering patient information, providing care, and communicating effectively with patients and team members. This position may involve collaboration within a team supporting multiple providers.

Individuals in this role will operate in an outpatient clinic environment.

Principal Responsibilities
  • Provides patient care support as delegated by Provider or RN
    • Administers therapies including medications, schedules follow-up visits, and manages prior authorizations or other specialty provider support.
    • Delivers patient care pre/post encounter or procedure, and offers preventative or other health information.
    • Collaborates with Provider and team to enhance and manage quality objectives.
    • May initiate, monitor, and discontinue IVs based on site requirements and training.
  • Rooms patients, manages patient flow, and collects adequate patient medical information/data.
    • Performs rooming and accurately records patients' vital signs and other necessary measurements in the medical record.
    • Coordinates communication of medical information and confirms details with patients when necessary, documenting per practice policy and standards.
    • Assists patients in preparing for physician examinations, providing psycho-emotional support as needed.
    • Facilitates the collection, labeling, and timely processing of specimens.
    • Assists physicians with procedures, including the preparation of medications as required.
    • Organizes patient flow to ensure physicians can move efficiently between rooms.
    • Conducts outbound calls to patients for follow-up as directed, which may include notifying patients of test results.
    • Accurately and thoroughly documents information in the patient medical record.
    • May check out and schedule follow-up visits with providers or appropriate staff before patients leave.
  • Receives or initiates patient phone calls.
    • Answers phones professionally and promptly.
    • May confirm or clarify patient symptoms using their own words.
    • Arranges appropriate appointments or forwards to RN or provider for review/recommendation.
    • Documents all encounters and patient interactions accurately in the electronic medical record.
  • Processes physician orders.
    • Performs clinical functions within the scope of license as ordered by the physician.
    • Responds promptly to patient needs and requests.
    • Reviews patient charts prior to visits to ensure availability of test results and therapy reports.
    • Provides appropriate instructions to patients as directed by the physician.
  • Maintains cleanliness, organization, and restocking of exam rooms.
    • Ensures proper maintenance and cleaning of all exam rooms and sterilizes instruments as trained.
    • Restocks required teaching supplies and ensures they are readily available.
    • Stocks and supplies exam rooms according to standards and patient needs.
  • Other duties as assigned.
Required Qualifications
  • Associate's or Vocational degree from a Board of Nursing approved LPN program
Preferred Qualifications
  • Two years of clinic LPN experience
Licenses/Certifications
  • Licensed Practical Nurse MN BON or Licensed Practical Nurse Wisconsin (if working in Wisconsin or interacting with Wisconsin patients remotely)
  • May require both state licenses based on the services provided at the location hired
  • Must meet at least ONE of the requirements below:

    •Current BLS certification from the American Heart Association

    •Current BLS certification from the American Red Cross

    •Allina in-house BLS training (within 30 days of hire)
