Nurse Navigator and Triage Professional:

We are seeking a skilled Nurse Navigator and Triage Professional to join our team at Weill Cornell Medical College. In this role, you will be responsible for navigating patients to the right site and time of care with the necessary urgency.

The ideal candidate will have 2+ years of experience as an RN, with a strong background in acute care, critical care, or emergency department work. Experience with telephone triage, Epic, and electronic triage software is highly preferred.

You will work closely with our clinical team to assess patient needs, provide guidance, and schedule appointments with the appropriate level of care. Your excellent communication skills, both written and verbal, will enable you to effectively navigate patients to the right site and time of care.

In this role, you will also participate in care coordination, reduce readmissions, and enhance chronic disease management. You will work collaboratively to manage health risks, assist patients in scheduling provider appointments, and communicate with providers as necessary.

This is a full-time opportunity with a competitive salary range of $80,000 - $110,000 per year, based on experience.
