Job Title: Geriatric Care Navigator

We are seeking an experienced Geriatric Care Navigator to join our team at Inspir Carnegie Hill. In this role, you will be responsible for navigating the complex healthcare system to ensure optimal care and services for our residents.

Job Responsibilities:
  • Navigate Healthcare System: Collaborate with healthcare professionals and family members to navigate the complex healthcare system, ensuring residents receive necessary care and services.
  • Assess Resident Needs: Conduct regular assessments to identify changing needs and adjust care plans accordingly.
  • Develop Care Plans: Create individualized care plans that address residents' unique needs and goals.
  • Maintain Effective Communication: Foster strong relationships with residents, families, and team members through regular communication and updates.
Requirements:
  • Nursing Degree: Minimum of a Bachelor's degree in Nursing (BSN) from an accredited institution.
  • Geriatric Experience: At least 2 years of experience working with older adults, preferably in a geriatric setting.
  • Strong Navigation Skills: Ability to navigate the complex healthcare system, identifying resources and services to meet resident needs.
  • Excellent Communication Skills: Strong verbal and written communication skills, with the ability to effectively engage with residents, families, and healthcare professionals.
Compensation and Benefits:

We offer a competitive salary range of $100,000 - $125,000 per annum, depending on experience. Additionally, you will enjoy a comprehensive benefits package, including health insurance, 401(k) matching, and paid time off.
