Healthcare Navigator

Alliance Homecare, a leading provider of long-term care in the NYC metro area, is seeking an experienced Registered Nurse to serve as a Healthcare Navigator. As a key member of our team, you will play a vital role in ensuring exceptional patient care and support.
In this full-time position, Monday to Friday, 9 AM–5 PM, with occasional on-call duties, you will work closely with patients, families, and healthcare providers to develop and implement personalized care plans. Your strong organizational skills, combined with your nursing expertise, will enable you to effectively coordinate care, manage resources, and provide holistic support to our clients.
We offer a competitive salary of $120,000 per year, reflecting the high value we place on your skills and experience. With a comprehensive benefits package, including 401(k) matching, health insurance, and PTO, we invest in your success and well-being.
To succeed in this role, you will need:
- A Bachelor of Science in Nursing (Master's preferred)
- An active, unencumbered nursing license in the state of New York
- Minimum of 5 years of nursing experience
- Strong communication and problem-solving skills
