Patient Navigation Specialist

The MCSP is a collaborative effort between Henry Street Settlement and New York Presbyterian Hospital to increase access to cancer screening and diagnostic services for uninsured individuals in Lower Manhattan.
About the Role
The Patient Navigator will report directly to the HSS Program Coordinator/Patient Navigator and collaborate with care teams to deliver patient-centered support services.
Key Responsibilities
- Schedule and confirm cancer screening appointments and ensure follow-through
- Assess social determinants of health and connect patients to internal and community resources
- Collaborate with providers and MCSP Case Managers to manage abnormal screening results
Qualifications
- Bachelor's degree in Social Work, Public Health, Nursing, or related field
- Certified in motivational interviewing and case management
- Proficient in databases and data entry (Foothold, Shared Google Drive, etc.)
