Job Summary:
We are seeking a skilled Healthcare Navigator to join our team at the City of New York. As a key member of our Public Health Clinics, you will play a critical role in promoting healthy communities and providing essential health services to New Yorkers.

Responsibilities:
- Oversee caseloads of STD clinic patients seeking HIV prevention services
- Deliver health education and promotion messages according to STD clinical protocols
- Coordinate logistics for plan adherence, including appointment reminders and medication adherence
- Collaborate with STD clinic staff to identify qualified candidates for programs

Requirements:
- A baccalaureate degree from an accredited college, including 12 semester credits in health education or health sciences
- 6 months of full-time experience in a health promotion or disease intervention program
- Strong communication and interpersonal skills

Benefits:
- Competitive salary: $**65,000-$85,000** per year, depending on experience
