Primary Care Navigator

We are seeking a dedicated and compassionate Registered Nurse to join our team at The Center for Alternative Sentencing and Employment Services (CASES) in New York, NY. This role offers an annual salary of $98,000, paid time off, 12 holidays per year, retirement plan, medical, dental, vision, and employee referral program benefits.
About the Role:As a Primary Care Navigator, you will work within a multidisciplinary group, providing comprehensive services to individuals with complex needs. Your primary responsibilities include delivering health assessments, counseling, psychoeducation, and medication management services to a caseload of participants. You will promote recovery, wellness, and optimal health while adhering to ethical and regulatory guidelines.
- Improve overall participant health through promotion of physical and mental health integration
- Conduct health screenings and provide linkage to primary and specialty care
- Perform blood draws, monitor and administer psychotropic injections, and prepare medication to be dispensed
- Work with external stakeholders to achieve positive client outcomes
Requirements include an active New York state license as a Registered Nurse, at least two years of experience working with adults who have mental health needs, and strong commitment to social justice and CASES' mission. We encourage individuals with lived experience to apply. Fluency in Spanish is a plus.
