Tandym Group is seeking a licensed Registered Nurse (RN) to join their growing team as a Healthcare Triage Coordinator in Suffolk County. This role involves providing exceptional care and support to residential programs, ensuring the well-being of consumers and staff.

Responsibilities:
  • Coordinate triage calls for all residential programs
  • Create and send Personalized Outreach Notes (PONS) as needed
  • Collaborate with Residential Managers and staff to coordinate urgent care and hospital visits, initiating doctor's orders in a timely manner
  • Maintain accurate medical records
  • Conduct assessments according to specific program regulations
  • Evaluate living conditions and provide resources for infection control
  • Develop and deliver in-services for staff, as required
  • Summon emergency services when necessary and follow up with medical appointments if warranted
  • Submit daily/weekend reports
Qualifications:
  • Degree from an accredited School of Nursing
  • Valid NYS Registered Nurse (RN) license

Desirable skills include experience working with individuals with disabilities.

Compensation:

The estimated annual salary for this position is around $65,000-$85,000, depending on qualifications and experience.
