Role Overview:
As a key member of the Care Management team, the Care Management Supervisor will report directly to the Manager of Care Management & System Navigation. This role is pivotal in ensuring the delivery of high-quality, patient-centered care.

Key Responsibilities:
1. Adhere to the standards and ethical guidelines set forth by the British Columbia College of Nurses and Midwives (BCCNM).
2. Implement a client and family-centered care approach that aligns with the vision and values of Providence Health Care.
3. Collaborate with the interprofessional team throughout the patient’s journey from admission to discharge.
4. Conduct thorough nursing assessments and guide the interprofessional team in care strategies and interventions.
5. Secure necessary funding and approvals for essential services.
6. Develop and enforce policies, procedures, and standards to enhance care management and discharge planning.
7. Engage in committees, task groups, continuing education, and corporate initiatives.
8. Utilize data to drive efforts, measure outcomes, and ensure the delivery of efficient, effective, and high-quality care.

Qualifications:
Education:
Current registration as a Registered Nurse with the BCCNM.

Minimum of three (3) years of relevant experience, including at least one (1) year in community care and discharge planning, or an equivalent combination of education and experience.


Skills and Competencies:
- In-depth knowledge of nursing theory and patient/family-centered care.
- Familiarity with BCCNM nursing standards.
- Proficient in evidence-based nursing practices for diverse patient populations.
- Understanding of pathophysiology and pharmacology relevant to safe discharge planning.
- Ability to apply medical appropriateness criteria using biological indicators.
- Knowledge of illness/disease processes and potential long-term complications.
- Awareness of the roles of other healthcare disciplines in patient care.
- Familiarity with clinical pathways, resource utilization, and patient assessment.
- Knowledge of external agencies and community resources.
- Skills in patient assessment, observation, and monitoring.
- Strong organizational and prioritization abilities.
- Proficient problem-solving, conflict resolution, and negotiation skills.
- Leadership and consultation capabilities.
- Ability to work independently and collaboratively within an interprofessional team.
- Excellent communication skills with clients, families, colleagues, and external agencies.
- Proficient in computerized client care information systems.
- Physical capability to perform job responsibilities.

Daily Responsibilities:
- Lead care coordination and discharge planning by working closely with the interprofessional team.
- Develop medical and functional goals, assign tasks, and ensure timely patient discharge.
- Facilitate patient flow by identifying those requiring specialized attention or alternative care levels.
- Serve as a resource for referrals related to diagnoses and post-discharge care.
- Monitor and evaluate patient progress, symptoms, and behavioral changes, adjusting care plans as necessary.
- Develop and recommend policies and procedures for effective care management and discharge planning.
- Consult with physicians, case managers, and the interprofessional team to address patient care issues.
- Act as a patient advocate, ensuring rights to autonomy, respect, privacy, dignity, and information are upheld.
- Provide support and information to families and other professionals regarding disease management and available resources.
- Participate in the orientation and training of new team members, fostering a culture of continuous learning.
