The Patient Service Coordinator plays a vital role in ensuring seamless care coordination and continuity for our patients. This position requires exceptional communication skills, strong organizational abilities, and the capacity to manage multiple assignments effectively.

  • Manage patient schedules, ensuring timely reassignment of visits and updates to agent unavailability in the worker console.
  • Initiate infection control forms as needed, sending completed reports to HRD for upload in the worker console.
  • Provide backup support during peak hours, receiving calls from field staff and assisting with weekly case conferences, referring clinical questions to the Branch Director as necessary.
  • Maintain the client hospitalization log, including coordination notes and electronic logs for office, field, and sales staff.
  • Complete requested schedules as tasks appear on the action screen, ensuring staff are scheduled for skilled nurse/injection visits unless an aide supervisory visit is scheduled in conjunction.
  • Process requested schedules for add-ons and applicable orders, including discharge visits, OASIS collections, and rescheduling declined or missed visits.
  • Run scheduling reports, including Agent Summary Reports and Missed Visits Done on Paper Reports.
  • Prepare weekly Agent Schedules, reviewing for productivity and geographic issues before forwarding to the Branch Director for approval.
  • Verify visit paper notes in the scheduling console as needed.
  • Assist with internal patient transfers between branch offices.
  • Perform lab report assessments, faxing copies to doctors and making copies for Case Managers, and routing to Medical Records.
  • Participate in on-call rotations and perform patient visits as required.

Required Experience/Skills:

  • Excellent communication and interpersonal skills, with the ability to interact with diverse individuals.
  • At least 1 year of home health experience.
  • Prior packet review/QI experience preferred.
  • Coding certification is a plus.
