Position Overview:
The role involves delivering comprehensive client management services, from assessing individual needs to coordinating referrals for a variety of identified services. These may encompass behavioral health support, youth development initiatives, mentorship opportunities, recreational programming, and additional services as necessary.
Key Responsibilities:
1. Conduct interviews with youth referred by various organizations to evaluate their behavioral health and social service requirements.
2. Identify suitable remedial services and formulate a detailed plan for the client, outlining the programs and services to be utilized.
3. Collaborate with supervisors to review recommendations before implementation.
4. Facilitate referrals to appropriate programs and services.
5. Maintain follow-up with clients to ensure the relevance of identified needs and confirm connections to referred services.
6. Monitor ongoing client participation in recommended programs until completion.
7. Assess the effectiveness of the recommended services and document case information for tracking and evaluation purposes.
Additional Responsibilities:
1. Prepare and manage confidential case records and associated files.
2. Foster collaborative relationships with the Department's Restorative Justice Staff, community providers, and various agencies.
3. Attend departmental meetings as required.
4. Operate general office and computer equipment.
5. Perform other related duties as assigned.
Supervision:
This position reports to the Director of Youth Services and receives guidance accordingly.
Essential Skills and Qualifications:
1. Ability to conduct thorough behavioral health and needs assessments with adolescents and their families.
2. Strong interpersonal skills to engage effectively with teenagers, parents, and professionals.
3. Capability to ensure youth and families are connected with appropriate services post-assessment.
4. Support for direct service programming as identified or developed internally.
5. Maintain and cultivate productive working relationships with other youth-serving organizations.
6. Uphold confidentiality in record-keeping.
Minimum Qualifications:
A Master's Degree in Social Work, Marriage & Family Therapy, or a related field is required. A minimum of one year of experience in a clinical behavioral healthcare setting with adolescents is preferred. Bilingual skills (Creole and/or Spanish) are strongly preferred.
