Position Summary: We are currently seeking a Functional Family Therapy (FFT) Family Specialist to join our dedicated team.
The FFT Family Specialist plays a crucial role in providing tailored case management support and services to meet the unique needs of children and foster families. This position involves close collaboration with the Family Therapist and the Department of Children and Families (DCF) to ensure stability within foster homes and facilitate permanency for children.
Key Responsibilities:
- Actively participate as a member of the FFT FC team, delivering home-based services alongside the FFT FC Therapist.
- Conduct ongoing risk assessments and interventions specific to the Foster Care model for each family, including personalized safety planning and crisis management.
- Ensure adherence to evidence-based practices and maintain accountability for model fidelity.
- Utilize a flexible, strength-based approach when engaging with youth and families.
- Address the clinical needs of clients through model-compliant FFT FC interventions.
- Perform initial assessments, reviewing referral information, engaging key participants, and analyzing family dynamics.
- Complete required documentation in Care4 and other electronic health records accurately and promptly.
- Facilitate supervised visitations for birth families, demonstrating respect for cultural diversity and heritage.
- Continuously evaluate engagement levels of all key participants and collaborate with supervisors and stakeholders to address barriers.
- Keep stakeholders informed of any elevated risks and attend family court appearances and Administrative Care Reviews.
- Identify and refer children and families to community-based resources as needed.
- Participate in video recordings for FFT consultation and certification as required.
- Engage in all model and agency training, peer supervision, and staff meetings.
- Independently manage a weekly schedule while keeping supervisors informed.
- Network with DCF and community organizations.
- Participate in on-call coverage on a rotating basis with program staff.
Qualifications:
- A Bachelor's degree in Social Work, Sociology, or Psychology is required, along with a minimum of two years of relevant experience.
- Experience in case management, coordinating care systems, and providing in-home services within complex family dynamics is essential.
