FAMILY SUPPORT COORDINATOR

We are seeking a dedicated and skilled Family Support Coordinator to become a vital member of our team at Homes for the Homeless (HFH). This role is essential in our City Sanctuary Facility for Families with Children, where you will be instrumental in guiding families through their journey, ensuring they receive the essential care and resources they require. Homes for the Homeless operates family shelters in New York City, providing more than just temporary housing. Our facilities offer a range of onsite programs designed to empower families and support their transition to stability. We strive to create a lasting positive influence on families, enabling them to thrive long after they leave our care.

KEY RESPONSIBILITIES

  • Maintain and manage both digital and physical records for all families in compliance with regulatory requirements.
  • Work collaboratively with families to create tailored service plans that address their unique needs.
  • Facilitate connections for families to community resources, support networks, and cultural services to enhance their social support.
  • Assist families in obtaining referrals to external agencies for legal, educational, parenting, medical, and mental health services.
  • Regularly update case files to document family progress and ensure accurate record-keeping.
  • Comply with all city and state regulations regarding documentation and record maintenance.
  • Conduct inspections of living units to ensure safety and compliance.

REQUIREMENTS

  • Bachelor's or Associate degree in a relevant field.
  • Proficiency in languages other than English, particularly Spanish, is highly preferred.
  • Familiarity with Microsoft Outlook, Word, and Excel.
