Position Overview
We are currently looking for a Field Nurse Care Coordinator to serve the North New Jersey and New York City regions. This role is primarily remote and home-based, requiring local travel to various healthcare facilities, including hospitals and clinics. The Care Coordinator will oversee the medical management of work-related injuries, which encompasses assessment, planning, coordination, implementation, and evaluation of care for injured or disabled individuals.

Key Responsibilities

  • Collaborate with insurance companies, healthcare providers, legal representatives, employers, and employees to ensure effective case management.
  • Monitor the recovery progress of injured workers and provide regular updates to employers and insurance carriers.
  • Utilize clinical expertise to develop and implement individualized care plans.

Qualifications

  • Current, unrestricted Registered Nurse (RN) license in NJ and NY.
  • Experience in managing workers' compensation cases is preferred.
  • Relevant certifications (CCM, CRRN, CDMS, CRC) are advantageous.
  • Strong organizational skills and proficiency in technology.
  • Excellent communication skills, both written and verbal.
  • Ability to work independently with minimal supervision.
  • Valid driver's license with a clean driving record.
