Compensation & Benefits: The salary range for this position typically falls between $52,000 and $57,000 annually, depending on relevant experience and qualifications. Organizational Mission: Community Health Action of Staten Island (CHASI) is dedicated to enhancing the health of New Yorkers by addressing food insecurity, supporting families affected by violence, and connecting individuals to essential healthcare services. CHASI offers outreach, education, prevention, and direct support to communities disproportionately impacted by health disparities.
Role Summary: The HIV Prevention Coordinator is responsible for implementing various outreach initiatives aimed at engaging individuals at heightened risk for HIV infection. Key responsibilities include:
- Conducting community outreach to establish connections with potential clients.
- Performing risk assessments to identify individuals at increased risk for HIV.
- Enrolling eligible clients into prevention programs and developing personalized service plans.
- Assisting clients in navigating their service plans, including referrals and appointment support.
- Providing education on prevention strategies, including the importance of regular HIV testing and safe practices.
- Linking clients to PrEP and PEP services and ensuring follow-up support.
- Supervising the Outreach and Education Specialist and peer workers, facilitating training and check-ins.
- Maintaining accurate documentation of outreach efforts and client engagement.
- Collaborating with internal teams and external partners to enhance service delivery.
Qualifications:
- A minimum of 3 years of relevant experience; a Bachelor's degree may substitute for some experience.
- High school diploma or equivalent required.
- At least 1 year of supervisory or program management experience.
- Experience in community outreach initiatives.
- Willingness to obtain Recovery Coach Certification within 6 months of hire.
- Ability to engage with individuals affected by substance use.
- Fluency in Spanish is highly preferred.
- Basic knowledge of HIV, HCV, STI, and harm reduction methods.
- Proficient in MS Office applications.
- Strong communication skills and ability to work collaboratively in diverse teams.
