Position Overview

The W Management Group LLC is in search of a dedicated and empathetic Resident Care Coordinator. This role is pivotal in ensuring that our residents receive the highest quality of care and support. We are looking for an individual who has a proven history of effectively managing services for residents while prioritizing their satisfaction and well-being.

Key Responsibilities:

  • Uphold the resident's rights as per facility standards.
  • Facilitate communication between residents and their healthcare providers.
  • Conduct assessments and manage the admission or re-admission processes from various care settings.
  • Oversee and coordinate clinical care for residents.
  • Ensure accurate and thorough documentation in line with facility protocols.

Qualifications:

  • A minimum of 1 year of experience as a Case Manager is required; experience in Long Term Care is advantageous.
  • Familiarity with Compuware systems is beneficial.
  • Strong communication skills, particularly over the phone, and the ability to engage with families and other care providers.
  • Excellent organizational skills, along with a genuine passion for serving the resident population.
  • Ability to collaborate effectively within a multidisciplinary team.
