Compensation & Benefits: The salary range for this position typically falls between $52,000 and $57,000 annually, based on relevant experience and qualifications. Community Health Action of Staten Island (CHASI) is dedicated to enhancing the health of New Yorkers by addressing hunger, supporting families affected by violence, and connecting individuals to essential healthcare services. CHASI focuses on outreach, education, prevention, and direct support for populations facing significant health disparities, including low-income individuals, those with chronic illnesses, and marginalized communities.
Role Summary:
The HIV Prevention Coordinator will oversee various initiatives aimed at reducing HIV transmission among high-risk populations. Responsibilities include:
- Conducting outreach to engage individuals at elevated risk for HIV infection.
- Providing education on behavioral practices and medical interventions that can mitigate HIV acquisition.
- Facilitating intake assessments and connecting clients to pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis (PEP) services.
- Collaborating with teams across CHASI's service locations to ensure comprehensive support for clients.
This role involves working closely with clients who are at high risk for HIV and may also be substance users. The Coordinator will provide educational resources, prevention materials, and direct service linkages to enhance client awareness of PrEP and PEP options. Additionally, this position includes supervisory responsibilities for the Outreach and Education Specialist and peer workers.
Key Responsibilities:
- Engage in community outreach to build relationships with potential clients.
- Screen individuals for risk behaviors associated with HIV infection.
- Enroll eligible clients in prevention programs and develop tailored service plans.
- Assist clients in executing their service plans, including referrals and appointment accompaniment.
- Educate clients on comprehensive prevention strategies, including safe sex practices and the importance of regular HIV testing.
- Provide information and referrals for PrEP and PEP, ensuring follow-up support.
- Maintain communication with clients who miss appointments to encourage adherence to their service plans.
- Supervise the Outreach & Education Specialist and peer outreach workers, conducting training and check-ins.
- Document outreach efforts and client engagement accurately.
- Participate in meetings and training sessions as required.
- Align all activities with CHASI's mission and values.
Qualifications:
- A minimum of three years of relevant experience is required; a Bachelor's degree may substitute for some experience.
- A high school diploma or equivalent is necessary.
- At least one year of supervisory or program management experience is preferred.
- Experience in community outreach is essential.
- Willingness to obtain Recovery Coach Certification within six months of hire.
- Fluency in Spanish is highly desirable.
- Knowledge of HIV, HCV, STI, PEP/PrEP, and harm reduction strategies is preferred.
- Proficiency in Microsoft Office applications is required.
- Strong organizational and multitasking abilities, with excellent communication skills.
