About the Role

As a Registered Nurse with HarmonyCares, you will play a vital role in providing high-quality, patient-centered care in the comfort of patients' homes. Our team is dedicated to delivering exceptional healthcare services, and we are seeking a skilled and compassionate nurse to join our team.

Key Responsibilities
  • Assess and evaluate patient needs, identifying mutually agreed-upon goals for care plans
  • Report patient status, updates, and discharge planning to clinical leadership, attending physicians, and hospice physicians
  • Attend staff meetings and educational in-services as needed
  • Participate in Quality Assessment and Performance Improvement (QAPI) program through submission of data collection
Requirements
  • Current unencumbered registered nurse in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)
  • Valid driver's license and good driving record
  • Ability to work in a field setting and exhibit sound nursing judgment
  • Ability to assess patient needs and formulate individualized patient care plans
  • Experience with Microsoft Office
