POSITION SUMMARY

The role involves a variety of responsibilities aimed at assisting adults with intellectual and developmental disabilities in their group-supported employment settings. Key tasks include providing transportation services using agency vehicles as well as personal vehicles, ensuring individuals are transported to and from their residences as needed.

KEY RESPONSIBILITIES

  • Collaborate with an interdisciplinary team to help individuals set and achieve their social, vocational, and behavioral objectives.
  • Facilitate individual participation in all aspects of program development and execution.
  • Deliver training to individuals to help them perform essential job tasks effectively and efficiently.
  • Provide ongoing support to enhance job performance and satisfaction.
  • Act as the primary liaison between individuals and their employers to promote workplace integration.
  • Ensure that the employer's production and quality standards are met, which may involve completing work tasks as necessary.
  • Employ approved behavioral management strategies to enhance individuals' inclusion in community work environments.
  • Evaluate the physical environment of service locations to identify and report potential hazards.
  • Maintain accurate documentation of individual progress and barriers to goal achievement.
  • Utilize technology for service documentation and tracking.
  • Advocate for accessible community resources by providing disability awareness information.
  • Assist individuals with personal care needs as required.

TRANSPORTATION DUTIES

Responsibilities include:

  • Driving agency-owned or personal vehicles to transport individuals to various locations.
  • Conducting pre- and post-trip inspections of vehicles and documenting necessary information.
  • Ensuring passenger safety through proper use of seat belts and wheelchair restraints.
  • Maintaining communication with the base station regarding service disruptions.
  • Keeping records of trips and documenting passenger behavior as needed.

QUALIFICATIONS

Successful candidates will possess:

  • A High School Diploma or G.E.D. with one to three years of relevant experience, or a bachelor's degree in a related field.
  • Strong communication skills, both verbal and written, for effective interaction with clients and documentation.
  • Basic mathematical skills for handling measurements and calculations.
  • Problem-solving abilities to address various challenges in the workplace.
  • A valid driver's license in good standing and the ability to secure a Public Service License within a specified timeframe.

PHYSICAL REQUIREMENTS

The role requires the ability to:

  • Stand, walk, and perform various physical tasks regularly.
  • Lift and move items weighing up to 25 pounds.
  • Assist individuals with mobility and personal care needs.

WORK ENVIRONMENT

Employees may encounter various conditions, including working near moving parts and exposure to outdoor weather. The noise level is typically moderate.
