RN Clinical Director, Home Health Specialist

CenterWell Home Health is seeking an experienced RN Clinical Manager to lead our home health care team. As a key member of our management team, you will be responsible for overseeing direct patient services, coordinating clinical operations, and ensuring high-quality patient care.
Responsibilities- Coordinate and oversee all direct care patient services provided by clinical personnel.
- Develop, plan, implement, analyze, and organize clinical operations for a specific location managed.
- Conduct assessments and reassessments of patients, including updating care plans and interpreting patient needs.
- Manage caregiver assignments and oversee the delivery of care to all patients served by the location.
- Review patient information, determine home health or hospice needs, and ensure patients meet admission criteria.
- Instruct and guide clinicians to promote effective performance and delivery of quality home care services.
- Monitor cases to ensure documentation compliance with regulatory agencies and requirements of third-party payers.
- Coordinate communication between team members, attending physicians, and caregivers to ensure appropriateness of care and outcome planning.
- Graduate of an accredited School of Nursing.
- Current state license as a Registered Nurse.
- Proof of current CPR certification.
- Valid driver's license, auto insurance, and reliable transportation.
- Two years of experience as a Registered Nurse with at least one year of management experience in a home care, hospice, or equivalent environment.
- Home care experience is required, preferably in home health.
- Management and people leadership experience are essential.
