Home Health Nurse Specialist

At PHH+, a Team Select Home Care Company, we are committed to providing exceptional in-home patient care with new methods of reducing re-hospitalizations and driving clinical outcomes. Our team is seeking a highly skilled Licensed Practical Nurse (LPN) to join our family of caregivers.
Job SummaryWe are currently hiring Private Duty LPNs for various counties across North Carolina, including Harnett, Johnston, Wake, Cumberland, Wayne, and Robeson. As an LPN at PHH+, you will have the opportunity to work directly with medically complex clients of all ages, providing one-on-one nursing care in their homes.
Responsibilities- Review, initiate, and implement care plans in accordance with client needs.
- Provide direct nursing care in the home, including medication management and administration.
- Monitor vital signs and medical equipment, performing physician-ordered interventions as needed.
- Educate clients and families while providing companionship and emotional support.
- Regularly assess clients' conditions, charting any changes and notifying the DON accordingly.
- Graduate of an accredited Nursing Program.
- Current, unencumbered RN license.
- BLS Certification.
- Pediatric experience preferred but not required.
- $30-$33 per hour salary.
- Flexible scheduling options, including days, evenings, overnights, and weekends.
