Primary Care Clinician

We are seeking a skilled Primary Care Clinician to join our team as a Home Visit Specialist. This role offers an exciting opportunity to work with patients in the comfort of their own homes, providing comprehensive care and support.

As a Primary Care Clinician with Optum, you will have the chance to make a real difference in people's lives by providing personalized attention and care. You will be working closely with our HouseCalls team to assess patients' medical needs, identify potential gaps in care, and develop tailored plans to improve their overall well-being.
Your Key Responsibilities- Conduct thorough home visits to evaluate patients' medical history, review systems, and perform physical exams.
- Identify diagnoses and develop care management plans to address patients' specific needs.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
- Educate patients on disease processes, medication management, and self-care techniques.
As a valued member of our team, you can expect a competitive salary range of $65,000-$80,000 per year, depending on your experience and qualifications. In addition to a generous compensation package, you will also enjoy:

- Ability to travel within assigned regions and occasionally beyond, up to 5% of the time.
