Clinical Care Provider for Home-Based Healthcare

This innovative model will provide annual clinical assessments to patients in their homes. This preventive care helps identify and reduce health risks for patients, coordinating follow-up care to improve health and well-being.
Main Responsibilities
- Conduct home-based assessments including past medical history review, systems evaluation, medication reconciliation, vital signs, and comprehensive physical exams
- Perform evidence-based practice screenings, including point-of-care testing as appropriate
- Identify diagnoses for care management and active medical management
- Formulate a list of current and past medical conditions using clinical knowledge and judgment
- Communicate assessment findings to inform primary care physicians of potential gaps in care
- Identify urgent and emergent situations and intervene appropriately
- Educate patients on disease processes, medications, and compliance
Job Requirements
- Active NP or PA license or ability to obtain by start date with unencumbered licensure
- National certification through ANCC, AANP, or NCCPA
- Ability to complete physical job requirements, including lifting 30-pound bags and navigating various dwelling conditions
- Reliable transportation for home visits
Salary and Benefits
- $40,000 salary with student loan repayment or $20,000 sign-on bonus for individuals without previous program participation
- No on-call, no weekends, and no holidays required
- Flexible scheduling with the ability to achieve work-life balance
- Student loan repayments cease if employee leaves Home and Community
- Full benefits of student loan repayments after 36 months in Advanced Practice Clinician or Physician role within Home and Community
