Clinical Care Specialist

Optum Home & Community Care is creating a new model in healthcare that addresses physical, mental, and social needs of patients.
We are uniting industry-leading solutions to build an integrated care model that helps patients access and navigate care anytime and anywhere. This includes annual clinical assessments conducted by our team members in the comfort of patients' homes.
This important preventive care identifies and reduces health risks for patients, coordinating appropriate follow-up care to improve health and well-being.
As a Clinical Care Specialist - Home Visit, you will have the opportunity to achieve work-life balance with flexible scheduling offered where providers can flex their time over the 7-day work week. No on-call, no weekends, and no holidays required.
Key Responsibilities:
- Conduct in-home assessments, including evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam
- Perform evidence-based practice screenings, including point-of-care testing (as appropriate)
- Identify diagnoses for care management and active medical management in furtherance of treatment
- Formulate a list of current and past medical conditions using clinical knowledge and judgment, along with assessment findings
- Communicate assessment findings to inform members' PCP of potential gaps in care
- Recognize urgent and emergent situations and intervene accordingly
- Educate members on topics such as disease process, medication, and compliance

Requirements:
- Active NP or PA license or ability to obtain by start date; licensure must be unencumbered, free of any open/unresolved disciplinary actions, including probation or restrictions against privilege to practice
- For NPs: Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification, or for PAs: Physician Assistant national certification through NCCPA
- Ability to complete physical requirements of the job, including lifting a 30-pound bag in and out of the car, navigating stairs, and various dwelling conditions, as well as sitting, standing, and kneeling for physical assessments
- Reliable transportation to complete home visit assessments
Benefits:
$40,000 Student Loan Repayment or $20,000 Sign-on Bonus for individuals who have not previously participated in this program. Flexible provider schedules available between 20-40 hours per week.
