Position Overview

AS A TARGETED CASE MANAGER, YOUR ROLE WILL INCLUDE...

  • Engaging with community members to facilitate integrated care solutions.
  • Evaluating clients' treatment requirements.
  • Creating, overseeing, and assessing treatment strategies and outcomes.
  • Encouraging collaborative efforts among various disciplines.
  • Delivering intensive case management for individuals facing co-occurring disorders.
  • Conducting assessments to identify individual strengths and needs.
  • Collaborating with clients to develop personalized, strength-based service plans.
  • Assisting clients in navigating obstacles to recovery, promoting independence and stability.

Qualifications

To be considered for this position, candidates should possess:

  • A minimum of a High School Diploma or GED with two (2) years of relevant experience in social services or a related field.
  • Alternatively, a Bachelor's Degree in social services.
  • A Pennsylvania Certification Board (PCB) certification (CAAP, CAAC, CADC, or CAADC) or be actively pursuing this certification during employment.
  • A valid PA Driver's License and the ability to travel for community-oriented services.
  • A collaborative mindset to work effectively with integrated team members.
  • Preferred knowledge or experience with trauma-informed care approaches.
  • Availability to work 40 hours per week within the agency's operational hours, which are Monday to Thursday from 8 AM to 8 PM, and Friday from 8 AM to 4 PM, with occasional Saturday hours.

Physical Requirements

Candidates must be able to:

  • See well enough to read reports and utilize a computer.
  • Hear adequately to communicate with clients, vendors, and colleagues.
  • Move about the agency and the community.
  • Lift up to 10 pounds.
  • Write, type, and effectively use technology.

Preferred Attributes

Ideal candidates will:

  • Have experience working with individuals with co-occurring disorders.
  • Be bilingual, with proficiency in both English and Spanish.
  • Possess knowledge of the 12 Step Program and community resources.
  • Have familiarity with trauma-informed care practices.
