Key Responsibilities:
- Engage with community members to promote Integrated Care.
- Evaluate clients' treatment requirements.
- Create, oversee, and assess treatment strategies and client progress.
- Encourage collaborative approaches among various disciplines.
- Deliver intensive case management for individuals facing co-occurring disorders.
- Conduct assessments to identify clients' strengths and needs.
- Assist clients in formulating a personalized, strength-based service plan.
- Support clients in navigating obstacles to recovery, fostering independence and stability.
Qualifications:
- A minimum of a High School Diploma or GED with two years of experience in social services or a related field.
- Alternatively, a Bachelor's Degree in social services.
- Possession of or actively pursuing a Pennsylvania Certification Board (PCB) certification (CAAP, CAAC, CADC, or CAADC).
- A valid Pennsylvania Driver's License with the ability to travel and provide community-based services.
- A collaborative mindset to work effectively with integrated team members.
- Preferred knowledge or experience with trauma-informed care.
- Availability to work 40 hours per week during agency hours, which are Monday to Thursday from 8 am to 8 pm, and Friday from 8 am to 4 pm, with occasional Saturday hours.
Physical Requirements:
- Vision: Must be able to see well enough to read reports and utilize a computer.
- Hearing: Must be able to hear adequately to communicate with clients, vendors, and colleagues.
- Mobility: Must have the ability to move within the agency and the community.
- Lifting: Capable of lifting up to 10 pounds.
- Other: Must be proficient in writing, typing, and using technology.
Preferred Attributes:
- Experience working with individuals who have co-occurring disorders.
- Bilingual candidates, particularly those fluent in English and Spanish, are encouraged to apply.
- Familiarity with the 12 Step Program and community resources.
- Experience with trauma-informed approaches is a plus.
