Job Overview

St. Paul's PACE program, designed for individuals aged 55 and older who are managing chronic health conditions or disabilities, is seeking a dedicated RN Case Manager. This program emphasizes coordinated healthcare to support seniors in maintaining their independence within their communities.

Position Highlights:

  • St. Paul's offers a competitive signing bonus for qualified PACE RNs (conditions apply).
  • This role is available for both LVN and RN candidates.
  • Full-time opportunity within the PACE Home Care division, with a standard schedule of Monday to Friday, 8:00 AM to 4:30 PM.
  • Reliable transportation is essential, as the role involves traveling to participants' residences using your own vehicle.

Key Responsibilities:

  • Under the supervision of the Home Care Coordinator, deliver in-home healthcare services to participants of Community Eldercare of San Diego (CESD).
  • Conduct initial and periodic assessments of participants in their home settings and collaborate on care plans with relevant resources.
  • Review and assess compliance with care plans, ensuring thorough documentation in participants' medical records.
  • Communicate any changes in participants' conditions and progress to the appropriate healthcare team members.

Qualifications:

  • Education: Graduate from an accredited nursing program with a current California RN or LVN license, CPR certification required; BSN preferred.
  • Experience: Minimum of one year working with elderly or frail populations, with relevant training or experience in the assigned role.

Physical Requirements:

Must be in good mental and physical health, capable of performing assigned duties. This includes a full range of motion for handling and lifting patients, manual dexterity, and coordination. The role requires standing, walking, pushing, bending, kneeling, and reaching for extended periods. Ability to assist participants requiring maximum support is necessary, including lifting up to 50 pounds and maneuvering objects over 150 pounds using proper techniques.
