Position Overview

St. Paul's PACE program (Program of All-Inclusive Care for the Elderly) in San Diego is dedicated to serving individuals aged 55 and older who are managing chronic illnesses or disabilities. Our innovative approach ensures that participants receive comprehensive medical care while maintaining their independence in their homes and communities. The healthcare teams at St. Paul's PACE focus on a collaborative model, integrating medical, social, and home care services to alleviate the burden of managing healthcare alone.

This opportunity is available for both LVN and RN candidates.



This is a full-time role within our PACE Home Care department, with a schedule of Monday to Friday, from 8:00 AM to 4:30 PM. This position necessitates reliable transportation, as travel to participants' residences using your own vehicle is required.

Key Responsibilities:

Under the supervision of the Home Care Coordinator, the RN will provide in-home support to participants of Community Eldercare of San Diego (CESD). Responsibilities include conducting initial and ongoing assessments of participants in their home settings, coordinating care plans with relevant resources, and reviewing care plans to ensure participant compliance. Accurate documentation of participants' medical records is essential, along with effective communication regarding changes in participants' conditions to the appropriate staff.

Qualifications:

Education - Must be a graduate of an accredited nursing program with a current California Registered Nurse License or LVN License. CPR certification is required; a BSN is preferred.Experience - A minimum of one year of experience working with the elderly or frail population is required. Relevant training or experience in the assigned role is essential.Physical Demands:

The candidate must be in good mental and physical health, capable of performing assigned duties. This role requires a full range of motion, including the ability to handle and lift patients, as well as manual dexterity and coordination. The RN must be able to stand, walk, push, bend, kneel, and reach for extended periods in various settings. The ability to transfer participants requiring maximum assistance is necessary, along with the capacity to lift/carry up to 50 pounds and push or pull objects exceeding 150 pounds, using proper body mechanics.
