Position Overview

St. Paul's PACE program (Program of All-Inclusive Care for the Elderly) in San Diego is dedicated to serving individuals aged 55 and older who are managing chronic illnesses or disabilities. Our mission is to ensure that these individuals receive comprehensive medical care, allowing them to maintain their independence within their homes and communities. The healthcare teams at St. Paul's PACE take a proactive approach in coordinating medical, social, and home care services, alleviating the burden of managing healthcare alone.

This opportunity is available for both LVN and RN candidates.



This is a full-time role within our PACE Home Care department, with a schedule of Monday to Friday, from 8:00 AM to 4:30 PM. Reliable transportation is essential, as travel to and from participants' residences using your own vehicle is required.

Key Responsibilities:

Under the supervision of the Home Care Coordinator, you will provide in-home care to participants of Community Eldercare of San Diego (CESD). Your duties will include conducting initial or annual/semi-annual assessments of participants in their home settings and coordinating care plans with appropriate resources. You will also review care plans and assess participant compliance, ensuring thorough documentation in their medical records. Effective communication regarding participants' changes and progress with relevant staff is essential.

Qualifications and Requirements:

Education - Must be a graduate of an accredited nursing program. A current California Registered Nurse License or LVN License, along with CPR certification, is required. A BSN is preferred.Experience - A minimum of one year of experience working with the elderly or frail population is necessary. Relevant training or experience in the assigned role is also required.

Physical Demands:

Candidates must be in good mental and physical health, capable of performing assigned tasks. This role requires a full range of motion, including the ability to handle and lift patients, as well as manual dexterity and coordination. The position involves standing, walking, pushing, bending, kneeling, and reaching for extended periods in various settings. You must be able to assist participants requiring maximum support and have the ability to lift/carry up to 50 pounds and push or pull objects exceeding 150 pounds using proper body mechanics.
