Position Overview

St. Paul's PACE program (Program of All-Inclusive Care for the Elderly) in San Diego is dedicated to providing comprehensive care for individuals aged 55 and older who are managing chronic health conditions or disabilities. Our innovative approach ensures that participants receive coordinated medical attention, allowing them to maintain their independence within their homes and communities. The dedicated healthcare teams at St. Paul's PACE offer a collaborative approach to managing medical, social, and home care services, alleviating the burden of healthcare management from participants.

Incentive: $3,000 signing bonus available for qualified PACE RNs (conditions apply).

This opportunity is open to both LVN and RN candidates.



This is a full-time role within our PACE Home Care division, with a work schedule of Monday through Friday, from 8:00 AM to 4:30 PM. Reliable transportation is essential, as the role involves traveling to participants' residences using your own vehicle.

Key Responsibilities:

Under the supervision of the Home Care Coordinator, you will deliver in-home support to participants of Community Eldercare of San Diego (CESD). Your duties will include conducting initial or periodic assessments of participants in their home settings and coordinating care plans with relevant resources. You will also review care plans and monitor participant adherence, ensuring thorough documentation of each participant's medical records. Effective communication regarding participants' changes and progress with the appropriate staff is crucial.

Qualifications:

Education: Must be a graduate of an accredited nursing program. A current California Registered Nurse License or LVN License, along with CPR certification, is required. A Bachelor of Science in Nursing (BSN) is preferred.Experience: A minimum of one year of experience working with elderly or frail populations is necessary. Relevant training or experience in the assigned role is also required.

Physical Requirements:

Candidates must be in good mental and physical health, capable of performing the assigned tasks. This role requires a full range of motion, including the ability to handle and lift patients, as well as manual dexterity and coordination. You should be prepared for prolonged periods of standing, walking, pushing, bending, kneeling, and reaching in various settings. The ability to assist participants requiring maximum support is essential, including the capacity to lift/carry up to 50 pounds and maneuver objects exceeding 150 pounds using proper body mechanics.
