MDS Nursing Coordinator

Key Responsibilities:
- Monitor and track the effective utilization of PDPM tools, including Physician Diagnosis Verification and various assessment tracking tools.
- Conduct monthly and quarterly analyses of QM and QASP performance, providing data-driven clinical recommendations to enhance facility outcomes.
- Deliver comprehensive PDPM and MDS training to newly appointed MDS Coordinators.
- Assess and determine the health status and care level of all new admissions, ensuring timely completion of MDS Assessments.
- Facilitate interdisciplinary collaboration in completing the Minimum Data Set (MDS) for each new resident in compliance with regulatory time frames.
- Ensure documentation meets federal, state, and medical standards, maintaining an accurate schedule of all MDS assessments.
- Oversee data entry processes to guarantee accurate electronic submission of MDS assessments and verify submissions as necessary.
- Coordinate resident care conferences, ensuring all MDS reviews are completed prior to these meetings.
- Assist in formulating and revising care plans, prioritizing resident problems and establishing realistic goals.
- Evaluate care plans for comprehensiveness and ensure they are reassessed and revised as needed.
- Manage all level of care changes within the facility, notifying relevant departments and generating necessary forms for processing.
- Maintain confidentiality of all resident care information, including protected health information.
Qualifications:
- Graduation from an Accredited School of Nursing with a Bachelor of Science Degree in Nursing preferred.
- Must be in good standing with the State Board of Nursing and fulfill all continuing education/licensing requirements.
- A minimum of 3 years of supervisory experience is preferred, along with long-term care experience.
Skills:
- Ability to make independent decisions and follow instructions effectively.
- Strong interpersonal skills to interact tactfully with personnel, residents, and families.
- Proficiency in Microsoft Suite products; knowledge of PCC is preferred.
Physical Demands: The role requires occasional lifting of up to 25 pounds and prolonged use of computer equipment. Reasonable accommodations may be made for individuals with disabilities.
Work Environment: The typical noise level in the work environment is low to moderate, conducive to focused work.
