Position Overview

St. Paul's PACE program (Program of All-Inclusive Care for the Elderly) in San Diego is dedicated to providing comprehensive care for individuals aged 55 and older who are managing chronic illnesses or disabilities. Our innovative approach ensures that participants receive coordinated medical services, allowing them to maintain their independence within their communities.

We are pleased to offer a $3,000 signing bonus for qualified PACE RNs (conditions apply).

This opportunity is available for both LVN and RN candidates.



This is a full-time role within our PACE Home Care department, with a schedule of Monday to Friday, from 8:00 AM to 4:30 PM. Reliable transportation is essential, as the role involves traveling to participants' residences using your own vehicle.

Key Responsibilities:

Under the supervision of the Home Care Coordinator, you will provide in-home nursing care to participants of Community Eldercare of San Diego (CESD). Responsibilities include:
Conducting initial and ongoing assessments of participants in their home settings.Coordinating care plans with relevant resources.Reviewing and evaluating participants' adherence to care plans.Maintaining accurate and thorough medical records for each participant.Communicating any changes in participants' conditions to the appropriate team members.Qualifications:

Education: Must be a graduate of an accredited nursing program with a current California Registered Nurse or LVN license. CPR certification is required; a BSN is preferred.Experience: A minimum of one year of experience working with elderly or frail populations is required. Relevant training or experience in the assigned role is essential.Physical Requirements:

The candidate must be in good mental and physical health, capable of performing the necessary tasks. This includes:
A full range of motion for handling and lifting patients.Manual dexterity and coordination.Ability to stand, walk, push, bend, kneel, and reach for extended periods.Capability to assist participants needing maximum support.Ability to lift/carry up to 50 pounds and push or pull objects exceeding 150 pounds using proper body mechanics.
