About the Role

Providence Care is seeking a dedicated Registered Nurse to join our team as a Case Manager for our Hospice program. As a Case Manager, you will play a vital role in providing compassionate care to patients and their families, ensuring their physical, emotional, and spiritual needs are met.

Responsibilities
  • Provide high-quality patient care, including assessing, planning, implementing, and evaluating patient care plans.
  • Coordinate care with a multidisciplinary team, including physicians, social workers, and other healthcare professionals.
  • Collaborate with patients and families to develop and implement individualized care plans.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions.
  • Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.
Requirements
  • Current RN licensure in the state of employment.
  • 1 year of experience in nursing, with a preference for hospice, home health, or palliative care experience.
  • Reliable transportation and a valid driver's license.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
