We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our team at Arkansas Hospice, Inc. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and support to our patients and their families.
Key Responsibilities- Manage patient care functions after initial admission and assignment to a designated RN Case Manager.
- Assist in the admission process as requested.
- Interpret and communicate Arkansas Hospice, Inc. policies and procedures to nursing staff and others to ensure understanding and compliance.
- Coordinate and collaborate with interdisciplinary team members to manage the plan of care on an ongoing basis to meet patient/family needs.
- Update the Plan of Care on an ongoing basis, keeping other team members current regarding the progress of the patient's physical status and any changing needs for care.
- Facilitate effective written and verbal communication with patients/families and other healthcare providers.
- Maintain communication with attending physician and community organizations concerning resources for assigned patients/families.
- Monitor pain and symptom control to ensure optimal patient comfort.
- Coordinate CNA scheduling needs with Patient Care Coordinator/Area Manager or designee.
- Initiate referrals for other hospice-related care and services.
- Serve as a preceptor for nurses in orientation, students, and trainees.
- Review documentation of staff as directed, ensuring documentation is complete, accurate, and timely according to organizational policies and practice standards.
- Participate in interdisciplinary team meetings, case conferences, care planning activities, and other committee meetings as assigned.
- Actively participate in Performance Improvement activities.
- Participate in community outreach activities to educate the public about Arkansas Hospice, Inc. services.
- Coordinate and provide education on hospice and hospice services for patients/families, other team members, and the community.
- Rotate on-call duty to provide 24-hour service to patients and families as assigned.
- Maintain strict confidentiality at all times.
- Utilize and model excellent customer service skills at all times, seeking opportunities to assist patients, families, and co-workers, demonstrating teamwork and cooperation.
- Accept direction and follow instructions from supervisor, seeking additional information as needed, working with minimal supervision.
- Adhere to all organizational and departmental policies and procedures.
- Continually meet organizational standards for attendance and punctuality, notifying supervisor in a timely manner when employee will be absent or late for work.
- Attend all required meetings and in-services, seeking opportunities for additional professional development activities as appropriate.
- Knowledge of palliative care with emphasis on symptom control associated with terminal illness.
- Knowledge of the holistic approach toward patient care and the nursing process as it applies to hospice patients and families.
- Knowledge and acceptance of hospice philosophy and principles of care.
- Skill in organizing and prioritizing workloads to meet deadlines.
- Ability to recognize both positive and negative aspects of death and grief.
- Ability to communicate effectively both orally and in writing.
- Ability to communicate effectively with co-workers and other customers.
- Ability to follow basic safety policies and procedures.
- Ability to use good judgment and maintain confidentiality of information.
