Position Overview: RN Case Manager
GiaCare is actively seeking qualified candidates for the role of a civilian Registered Nurse Case Manager.
Qualifications:
- Education: A Bachelor of Science in Nursing (BSN) is required.
- Experience:
- A minimum of two years of full-time clinical nursing case management experience is essential.
- Demonstrated expertise in key Case Management functions, including assessment, planning, facilitation, and advocacy.
- Familiarity with HIPAA regulations, AAAHC, and TJC standards is necessary.
Certifications:
- Basic Life Support (BLS) certification is mandatory.
- One of the following certifications is required: CCM, CDMS, CRRN, COHN, COHN-S, ACCC, CRC, RN-NCM, CMC.
Licensure: An active and unrestricted RN license is required.
Key Responsibilities:
- Engage in all facets of the Case Management Program.
- Perform patient assessments and create comprehensive care plans.
