Description:
Under the guidance of a healthcare provider, the Patient Care Coordinator engages in a range of patient support tasks while adhering to the performance standards of all designated services within the clinics of Trinity Health.

Key Responsibilities:
- Evaluate patient records for thoroughness and history.
- Prepare examination and procedure areas along with necessary equipment for patient care.
- Welcome and identify patients, ensuring their readiness for examinations while maintaining confidentiality.
- Review and document current medications and allergies.
- Measure vital signs, gather chief complaints, and document patient history.

Hours/Schedule:
40 hours biweekly (0.5 FTE)

Essential Qualifications:
- High School Diploma or GED
- Valid LPN license, RN license, or Medical Assistant Diploma
